Dr. Fauci in New England Journal of Medicine: Coronavirus less severe than the flu

Chief medical advisor to the Trump administration, Dr. Anthony Fauci’s recent findings have just been published in the New England Journal of Medicine, the world’s leading medical journal. Published continuously for over 200 years, NEJM delivers high-quality, peer-reviewed research and interactive clinical content to physicians, educators, and the global medical community.

“In another article in the Journal, Guan et al. report mortality of 1.4% among 1099 patients with laboratory-confirmed Covid-19; these patients had a wide spectrum of disease severity, If one assumes that the number of asymptomatic or minimally symptomatic cases is several times as high as the number of reported cases, the case fatality rate may be considerably less than 1%.

This suggests that the overall clinical consequences of Covid-19 may ultimately be more akin to those of a severe seasonal influenza (which has a case fatality rate of approximately 0.1%)  or a pandemic influenza (similar to those in 1957 and 1968) rather than a disease similar to sSARS or MERS, which had case fatality rates of 9 to 10% and 36% respectively.”

Fauci’s been saying 1% from the outset, as I recall. You just won’t find any journalists reporting that.

So now he is saying that “the case fatality rate may be considerably less than 1%”. But even if it is 0.2%, that would still be twice as bad as flu [0.1%]. And I don’t know if Fauci ever challenged the huge 3.4% that the WHO is using.

Our observed CFR at the moment [afternoon, 3/28] is about 2%, but we know there are lots of unconfirmed cases. The WHO number is plausible as the death rate for those who are tested positive.

But Fauci didn’t “just” write it, since the statistics in the article are end-of-February. I suspect his views have narrowed, in that the error bar on whatever estimate he wishes to make are less.

Fauci, a member of the Coronavirus Task Force, said he thinks Trump was making “an aspiration projection to give people some hope.” But with the cases still increasing “dramatically” daily, Fauci said it is not yet “time to pull back.”

“Well, I think what the president was trying to do, he was making an aspirational projection to give people some hope,” Fauci said on CNN. “But he’s listening to us when we say that we’ve really got to reevaluate it in real-time, and any decision we make has to be based on the data. … When you have a situation where the cases today compared to tomorrow is increased dramatically and then the next day is increased dramatically, that’s no time to pull back. That’s when you’ve got to hunker down, nail down, mitigate, mitigate, mitigate, get the people taken care of. That’s what you’ve got to concentrate on. You have to go with the date.”

I’m liking this guy less and less. These appointees are great guys until they aren’t. Robert Mueller was a great guy at first, Jeff Sessions was a great guy at first. Bill Barr, time will tell.

David Knight of Infowars has resurrected impeachment with—-IMPEACH ‘PRESIDENT’ FAUCI, and I agree.
We would be better off if Trump wasn’t intimidated by Dr. Fauci and would follow his own instincts to put Hydroxychloroquine at the top of the list for ramped-up production and distribution to treat COVID19 positive patients, esp. those at high risk. As a retired doctor, I’m more interested in the availability of Plaquenil and Z-pak rather than respirators which hopefully we wouldn’t need if docs weren’t intimidated and follow their instinct to prescribe it for patients who want it and who don’t have any contraindications. NO GOVERNOR, NIH, CDC, FDA or a ‘President’ Fauci can prevent a doctor from prescribing established drugs “off-label” given the fact that this drug has been successfully used on patients in China, South Korea, France, Belgium and Bahrain infected with COVID19. You don’t have to go through hoops of using an experimental drug in a controlled trial…which means patients who are sick might have to get a placebo. Docs shouldn’t hoard the drug or prescribe it for anyone other than someone who has a known chronic clinical indication (like lupus or Rheumatoid Arthritis) or for anyone positive for COVID19 which is what I would do if still in practice.

I feel like I’m one of Dr. Fauci’s laboratory rats…let’s study to see how this disease spreads w/o intervening directly to kill the virus. Let’s separate the rats and see if we can control the spread. And, when we get around to it, let’s see how this or that drug controls the disease…and when we get around to it…we’ll publish the double-blind controlled study in a peer reviewed journal. Then we’ll immunize the rat population and see how many die or get sick from our vaccine.

I truly don’t get it, Brooke. This is the exact same argument antivaxxers use to minimize measles and polio deaths: by adding in all the asymptomatic seroconversions.

Since there’s no vaccine for the novel coronavirus, it’s possible we would have 100 million (or more!) infections, counting the minor and asymptomatic ones. That’s 100,000 excess deaths even at the 0.1% rate, but if the true CFR were 0.5%—we are still in the educated guess stage at best on this statistic—it would be a truly disastrous 500,000.

As for the headline “less severe than the flu”, that is literally nowhere in the original article. The 0.1% rate is equal to that of a severe flu season. It’s much lower than MERS or the original SARS, but that’s compensated for by a much longer infectious period before symptoms.

USA deaths continue to double every 4-5 days, no end in sight.

On CNN on March 29, Fauci predicted up to 200,000 dead from the coronavirus. That’s a lot worse than the flu, except maybe for the 1918 epidemic.
